IV Direct: Initial dose 20 mg, may repeat 20-80 mg at 10 minute intervals until desired supine BP or total of 300 mg has been given. 2. Infusion: Always use an infusion pump. The infusion may be initiated at 0.5 mg/minute (up to 2 mg/min) and increased by 0.5 mg/min every 15 minutes until desired BP goal is achieved. Slow direct IV push administered over 2 minutes. Loading infusions (2 mg/min) require close monitoring of HR and BP and are terminated after response occurs or a cumulative dose of 300 mg is reached. Continuous infusions of 2-6 mg/hr have been used in some settings and should not be confused with loading infusions. Storage -The effective IV dose is usually in the range of 50 to 200 mg.-A total dose of up to 300 mg IV may be required in some patients.
